If you are:

  • a Ukrainian citizen or a spouse of a Ukrainian citizen and came to Poland from Ukraine after 23 February 2022 in connection with the war,

  • a Ukrainian citizen or a spouse of a Ukrainian citizen and you lived in Poland before 24 February 2022,

and you want to submit an application for the ‘500+’ child benefit or family care capital for a child that came to Poland from Ukraine after 23 February 2022 in connection with the war, you can do it on a form specially prepared for you.

You can fill in the application and submit it electronically to the Social Insurance Institution (ZUS) through its Electronic Services Platform (PUE) – i.e. the Social Insurance Institution’s electronic system.

The information from when and how you can submit the application will be soon available on the following website www.zus.pl.

In order to submit the application to the Social Insurance Institution, you will need:

  • Polish identification number (PESEL) for you and the child,

  • e-mail address,

  • Polish phone number,

  • bank account number in Poland.

The registration of Ukrainian citizens aimed at assigning them the Polish identification number (PESEL) started in commune, municipal or district offices in Poland on 16 March 2022. The PESEL number assigned as of 16 March has the marking confirming that you or your child came to Poland from Ukraine after 23 February 2022 in connection with the war.
